- The face amount is greater than $5,000,000.
- More than one policy owner is involved.
- If term riders other than Child Term Rider, Accidental Death Benefit, Waiver of Premium, and Income Provider Option are desired.
- Application is for a policy change.
- The application is for premium financing.
- The Proposed Insured is less than 18 years of age.
